Project Concept

The central concept of the JC Pathways to Healthy Aging focuses on increasing the preparedness of Hong Kong for the population aging. Aligning with the advocacy of World Health Organization to develop and preserve the functional ability of older adults, the JC Pathways to Healthy Aging is underpinnied by five concepts:

1. Reduce health disparity

As various physical and psychosocial factors would accelerate the aging process and functional decline, the JC Pathways to Healthy Aging will target this high-risk group for proactive and early intervention.

2. Comprehensive health diagnostics

Aligning with the global initiative to promote healthy aging, our project adopts the “Integrated Care for Older Adults” approach to assess the intrinsic capacity to guide care planning.

3. Critical pathway to provide standardized person-centered care

Adopting the critical pathway to standardize the process of healthy aging promotion with consideration to individual intrinsic capacity of older adult.

4. Empowerment based healthy lifestyle promotion

In addition to provide the knowledge and skill set of older adults to optimize functional ability, goal-oriented empowerment strategies will be used deliberatively to sustain healthy behavioral changes.

5. Capacity building

To translate the JC Pathways to Healthy Aging as an contextual-specific community initiative to tackle the population aging in the coming decades, the project develops the human resource
(including volunteering health coach and formal care workers) and infrastructure (I,e, toolkit) to sustain the program delivery.
