Pathway to Healthy Ageing Project
The issue of population aging is becoming increasingly severe. According to the World Health Organization, by 2030, one in six people worldwide will be over the age of 60. By 2050, Hong Kong will become the city with the highest degree of population aging in the world.
In view of this, the World Health Organization has proposed a global initiative called the “Decade of Healthy Aging 2021-2030” to promote innovative service models for healthy aging by mobilizing national governments, community organizations, international agencies, professionals, and academics.
As a result, the University of Hong Kong School of Nursing has designed the “Jockey Club Pathway to Healthy Ageing Project?” with the aim of identifying high-risk older adults in the community experiencing aging-related decline, providing them with health assessments recommended by the World Health Organization, and developing personalized, multifaceted health promotion measures.
What is our plan?
NEWS & EVENTS
1. Responding to the UN’s Declaration on the Decade of Healthy Ageing (2021-2030), The Jockey Club Pathway to Healthy Ageing (JC-PATH-HA) has been launched since July 2021 to tackle accelerated ageing. Based on the territory-wide healthy ageing diagnostics of over 12000 older adults using the WHO-Integrated Care for Older People (ICOPE) model, this nurse-led person-centered, empowerment-based critical care pathway improved the functional status for our seniors. Through disseminating the implementation and impact of the JC-PATH-HA, the Symposium will stimulate discussion on the re-engineering of the community care service for elderly in Hong Kong
2. We are currently recruiting social workers, social worker assistants, and staff from elderly centers within social welfare organizations to participate in the Jockey Club Pathway to Healthy Ageing Project’s social worker training programme.
