The Symposium will be officiated by Mr Ho Kai Ming, JP, Under Secretary for Labour and Welfare Department and Dr. Donald Li, JP, Chairmen of the Hong Kong Elderly Commission.  Professor Lau Chak-sing, Dean of LKS Faculty of Medicine, HKU will deliver the Welcoming Remarks.

The JC Path-HA Program has rolled out as an exemplar model in response to the United Nation advocacy on 2021-2030 Decade of Healthy Aging.  The initiative is supported by the Hong Kong Jockey Club Charities Trust. It has been commenced in July 2021 in collaboration with 14 District Elderly Care Centre (DECC) and Neighborhood Elderly Centre (NEC) in 7 districts operated by three flagship NGOs in HK. The JC Path-HA Program aims to optimize the functional status of older adults according to the WHO Integrated Care of Older People (ICOPE) model.  Over the years, we have conducted the WHO-ICOPE functional assessment (covers physical, psychological, cognitive, metabolic vitality, sensory function) to over 10,000 older adults from the 18 districts in Hong Kong.  Among this group, a nurse-led critical care pathway was used to provide person-cantered functional optimization to over 2,300 older adults with impairment.

In the symposium, our team will present the findings on healthy aging diagnostics in HK based on the territory-wide spatial analysis, the overall design of JC Path-HA Model and the results of impact evaluation.  By using the Asset Based Community Development Approach, the Program have trained more than 400 health coaches (late middle age and older adults) to take part in the case management for healthy aging promotion. They are involved in conducting health assessment, deliver semi-professional health promotion activities in the community and coach lifestyle modification.  We are now working with the collaborating NGOs to provide social care worker training as well. With the continuous evolvement of primary healthcare in our city, we hope that the symposium will stimulate interactive dialogues among the aged care stakeholders on how to advance aging preparedness in Hong Kong and beyond through policy setting, service re-engineering and strategic research endeavors.
