Background

Elderly people are often considered frail and sick, and a burden on society, but is this really the case? In fact, an extended life expectancy not only provides new opportunities for the elderly and their families, but also allows them to contribute to society. For example, seniors can spend more time with their children, choose further education, engage in new careers, or participate in volunteer activities. Elderly people can still contribute to their families and communities in various ways. However, these opportunities and contributions depend on a very important factor: health!

Therefore, the World Health Organization has developed the “Decade of Healthy Ageing Action Plan”, and the “Jockey Club Pathway to Healthy Ageing Project” was also launched to promote healthy

Project Design

The project used a ‘pathway’ as the metaphor to illustrate a systematic method to optimize the functional status of older adults through person-centered care and sustainable lifestyle changes. The pathway has three stations guide the at-risk older adults who has functional impairment.

Scope of Services

Health Screening

TheWHO’s Integrated care for older people ( ICOPE) guideline was used to assess if the older adults with risks of accelerated ageing including frailityfraility,sarcopenia, malnutrition, social loneliness, mood disturbance and other geriatric symptoms.

Health Coach Training

A series of trainings were provided to the health coaches by nurses and social workers in the project so that they understand the common health problems among the older people.

They attended small class discussion and trainings with the team nurses and social workers on how to closely monitor cases and provide advice to the older people.

Health Ageing Workshop

The project consists of nine cohorts (Three months as one cohorts) and implement in two phases.

Phase 1: The nurse team, social workers and health coaches helps the elderly clients to tailor made health plans accordingly to their personal needs and health problems.

Phase 2: The workshops, chatroom and broadcasts were organised. A series of educational workshop run by health coach to educate older adult adultforimproving their intrinsic capacity and building up a healthy lifestyle. The health topicsincludcludehealthy eating, health fitness, fall prevetion, pain management, insomnia, mental wellness and healthy brain. The participant participants with needsattendattendedvarious health promotion activities to develop knoweldge and skills for healthy behavioral changes changes.
